What are the office hours?
The doctors office is open from 8AM - 4:00PM Monday through Thursday.
What should I bring?
Please bring with you to your appointment the following information:
MRI and/or X ray results
Picture ID and Insurance card
List of current medications
Patients who reside in a Skilled Nursing Facility and those under 18 y/o need to be accompanied by a family member.
How do I refill a prescription for medicine?
Call our office 734-464-0400 extension 842. Please leave your name, with correct spelling, date of birth, your OIM physician, what drug you need refilled, and pharmacy phone number. Please allow 24-48 hours for processing. Refills are not processed on the weekend.
How can I pay my copay?
Copays are due at time of service. We accept cash, check, Visa and Mastercard.
What do I do if I want to get my records from you?
Your records and x-rays are kept in strict confidence as part of our permanent file. We will release copies only if we have your written permission. For copies of patient records or x-rays we must charge, to cover our material costs. Please allow us at least 5 business days to copy your records.
The cost to copy records is $20, plus $1 per page. Records requested by you and picked up by you, do not need to have a signed authorization form.
Another doctor may request your records, however, you must sign a release form authorizing us to fax your records to that physician.
The charge for copying x-rays is $25, paid in advance. The x-rays are digital and will be copied to a disc.
Can you fill out a disability form?
We will be happy to complete the appropriate form you bring to our office, but there is a charge to defray our administrative costs. The charge is $15 to complete your form. Please allow us 7 business days to complete your form.
What if my attorney requests records or forms?
If your attorney needs reports or forms, we'll be happy to respond to his or her written request, but only with your signed approval. Depending on the request, all charges need to be prepaid by either you or your attorney.
Can I see my OIM physician the same day as I have physical therapy?
Yes. Most insurances allow you to see the physician and have physical therapy on the same day. However, it is advised that you schedule your physical therapy appointment first.
I just received my bill and it is several pages long. How come?
All services performed in our office are listed on one bill. We no longer send you a separate bill for services performed in the doctors office and physical therapy.
